	Tourist and Business Visa Requirements

Travel notes:

	Us passport must be signed, valid for at least 6 months and with at least 2-3 blank visa pages not amendment pages in passport. If your passport does not meet the requirements, we can assist with obtaining new passport, passport renewal or add additional pages for a valid US passport.
	Application forms may be duplicated on plain paper but must be clear copies with original signature.

Entry requirements:

Passport and Visa are required.

Tourist and Business Visa requirements:

	Valid, signed US passport.
	No Tourist Visa for visit less than 20 days, longer visits require visa.
	Please note: Bermuda residents must use Washington application
	One Visa Application (for New York) or One Visa Application (for Washington D.C.) Form completed and signed. Please note application must be NOTARIZED.
	If staying more than 59 days you must photo copy actual airline tickets and original most recent bank statement.
	Please note that you will need Adobe Acrobat Reader version 3 or higher to view the form.
	One passport type photograph.
	Copy of flight itinerary.
	Visa Order Form.
	If travel is for business please include a business letter of responsibility, click for Sample business letter.
	For business letter please note the following:
	This letter must be on your company letterhead. This letter must be sign by someone other than the applicant.

Health Requirements:

Vaccinations are NOT required unless travel is from or through an infected area.

	Consular fee and processing time: (consular fees may change)
	$ 30 Per Tourist/Business Visa 3 business days MINIMUM processing time/Single entry.
	$ 50 Per Tourist/Business Visa 3 business days MINIMUM processing time/6 month multiple entries.
	$ 75 Per Tourist/Business Visa 3 business days MINIMUM processing time/12 month multiple entries.

Shipping fees:

	Federal Express starting at $25.95 for priority Weekday Delivery. Other Special Deliveries of Federal Express VARY (Ex. SATURDAY DELIVERY OR 8AM DELIVERY, call for rates). RESIDENTS deliveries is additional $3.00 to have signature required. Verify SIGNATURE OR NO SIGNATURE must be selected.
	Use Federal Express, Airborne, UPS, or DHL Priority overnight for delivery by 10:30 AM to ship all your passport application materials to:
