SECOND PASSPORT INSTRUCTIONS
  NEW UPDATES: AS OF NOVEMBER 4, 2009 ALL applications presented to acceptance facilities or JIDS MUST be done through the www.travel.state.gov website utilizing the bar-code, then printed, signed and given to Just-In Document Svc.or to acceptance facility to seal, then JIDS to process. Also note a 2 passport size 2x2 passport quality photos with a white background must accompany the request and a new booklet will be issued, and the old one will be returned. This is in addition to the requirements listed. ANY application HANDWRITTEN, received by JIDS will be redone with bar-code by JIDS and charged a service fee of $25.00 in addition to processing fees. If applying for a duplicate passport due to a conflict of interest in Middle Eastern countries. You should use FORM DS-82, follow the instructions for passport renewal with a letter on company letterhead (click here for sample letter) explaining the conflict of interest. Example: Going to Saudi Arabia and Israel.

If applying for duplicate passport to facilitate visas for business travel. You should use FORM DS-11 at an acceptance facility follows instructions for 1st time passport and includes the business letter on company letterhead (click here for sample letter) addressed to US Department of State explaining the need to expedite travel and facilitate emergency visas.
 
Step 1: Collecting US passport application materials:
 
  1. Your valid US passport:
     
    It will be returned with the new passport.
     
  2. Letter to US Passport Services:
     
    The letter must state the reason for obtaining a second passport. Click here for sample letter. Letter must be signed by the traveler
    Please note: The passport agency has advised if the passport has not been used for travel to the Middle East and is being renewed, the letter must state the circumstances. The US Dept of State will make the decision to issue a new booklet based on travel over the previous two years.
   

Call the office for instructions if issuing this passport to facilitate visas instead. To be eligible your original passport must be getting visas and you must go to an acceptance facility with birth certificate and photo identification as well as other supporting documents.
     
  3. Two Passport Photos:

  4. Letter of Authorizations:

  5. US Passport Application Form:
     
      Passport services will no longer accept DOUBLE SIDED applications printed from your computer. Double sided is only accepted on ORIGINAL applications which can be obtained from an acceptance facility, such as post office.

    WHEN FILING APPLICATION on any DS 11,82 or 4085. In MAIL TO: must be Just-In Document Service, 5 W. Main St., Elmsford ,NY 10523. In permanent is where the applicant puts there own home address. A $25.00 fee will be charged if this is not done correctly.
 
  5. Proof of Travel within 14 days (one of the following):
     
    Copy of flight itinerary from the travel agent or airline.
    Copy of plane ticket.

Step 2: Authorization Process:
    A $ 127.00 (A $ 12.00 Security fee is being added to the original fees $ 115.00) company check or money order with the name and date of birth written on it issued to US DEPARTMENT OF STATE, or Just-In Document Service, Inc can issue a check for you and charge your credit card $ 5.00 check writing fee. Step 3: Ship application materials via Federal Express:
Use Federal Express, Airborne, UPS, or DHL Priority overnight for delivery by 10:30 AM to ship all your passport application materials to (Please include label for UPS/DHL account a $5.00 label fee if not included with request.) :
