NEW (Child) PASSPORT INSTRUCTIONS
NEW UPDATES: AS OF NOVEMBER 4, 2009 ALL applications presented to acceptance facilities or JIDS MUST be done through the www.travel.state.gov website utilizing the bar-code, then printed, signed and given to Just-In Document Svc.or to acceptance facility to seal, then JIDS to process. Also note a 2 passport size 2x2 passport quality photos with a white background must accompany the request and a new booklet will be issued, and the old one will be returned. This is in addition to the requirements listed. ANY application HANDWRITTEN, received by JIDS will be redone with bar-code by JIDS and charged a service fee of $25.00 in addition to processing fees. Step 1: Authorization Process:
    Please note that as of February 1st, 2004 ALL customers using DS-11 must appear in person at an acceptance facility to expedite passport request, INCLUDING CHILDREN OF ALL AGES. Previously children under 13 years of age did not have to appear, this has changed as an additional step in the prevention of parental child abduction and substitution.
     
    As of February 1, 2008. The US passport Applications for CHILDREN under AGE 16 REQUIRE BOTH PARENTS’ CONSENT. This new requirement will apply when either form DS-3053, Statement of Consent, or a complete written consent letter, is submitted with an application for a child under the age of 16.
     
   

Present the following IN PERSON to your local Passport Acceptance Agent (in local Post Office or Court Houses):
      ANY SEALED ENVELOPES:Client must make copy of application that enclosed in sealed envelope COPY MUST BE ON OUTSIDE OF ENVELOPE effective immediately.
      Passport services will no longer accept DOUBLE SIDED applications printed from your computer. Double sided is only accepted on ORIGINAL applications which can be obtained from an acceptance facility, such as post office.
   
  • Proof of travel within 14 days.
  • Proof of US citizenship.
  • Passport application form DS-11 completed online but NOT signed as the it should be witnessed by the Acceptance Agent.
  • Two passport photographs.
  • One copy of authorization letter.
  • Actual plus clear photocopy of driver's license with photo on it.
  • Government fees: $165.00 ($25.00 cash to the Acceptance Agent + $140.00 check or money order to US DEPARTMENT OF STATE).
    The Acceptance Agent endorses the Passport Form (DS-11) and place it in a Government envelope which should NOT be opened after it is sealed. (Envelope will not be sealed without a 14 days proof of travel). Sealed envelopes from the US Dept of State must be presented within 5 business days of being sealed. JIDS fees will be based upon available appointments and the fees will range from 120.00 to 255.00
 
Step 2: Collecting US passport application materials:
  1. Proof of US citizenship: (one of the following)
     
    Original or Certified copy of Birth Certificate (if born in the US).
    Old passport of which the applicant was the bearer.
    Original Naturalization Certificate (US citizens through naturalization).
    Certificate of Citizenship (if born abroad to US parent/s).
     
  2. Parents Proof of Identity:
     
    Valid driver's license with photo issued more than 6 months ago.
    A clear photocopy of driver's license with photo on it.

3. Child's Relationship to parent(s)/Guardian: (one of the following)
     
    Original or Certified copy of Birth Certificate (if born in the US).
    Foreign Birth Certificate (with parents names & official translation).
    Adoption Decree (with adoption g parents names).
    Court order establishing Custody OR Guardianship.
     
  4. Parental Application Permission Documentation: (one of the following)
     
    Please note the child should appear in person under all cases.
    Both parents must appear together.
    One parent appears, signs, and submits second parent's notarized statement of consent authorizing passport issuance for the child (a notarized Form DS-3053, Statement of Consent: Issuance of a Passport to a Minor Under Age 16, or a notarized written statement with the same information on a sheet of paper from the non-appearing parent).
    One parent appear and submits primary evidence sole authority to apply:
     
  • Original or Certified copy of Birth Certificate listing only applying parent.
  • Foreign Birth Certificate (official translation) listing only applying parent.
  • Court order granting sole custody to applying parent.
  • Adoption decree, if applying parent is sole adopting parent.
  • Death certificate of non-applying parent.
  • If none of the above documentation is available, the applying parent/guardian should submit a signed statement explaining why the non-appyling parent/guardian consent cannot be obtained.

  7. US Passport Application Form:
     
    US passport application form DS-11 can be completed online or picked up at a Passport Acceptance Facility.
    You will need to sign form DS-11 front of Acceptance Agent.
    Sealed envelopes must have a copy of the application on outside of envelope proving the application has a bar-code on top left corner, if it does not JIDS will redo application and charge a $25.00 Service Fee.
    WHEN FILING APPLICATION on any DS 11,82 or 4085. In MAIL TO: must be Just-In Document Service, 5 W. Main St., Elmsford ,NY 10523. In permanent is where the applicant puts there own home address. A $25.00 fee will be charged if this is not done correctly.
 
  8. Proof of Travel within 14 days (one of the following):
     
    Copy of flight itinerary from the travel agent or airline.
    Copy of plane ticket.

Step 3: Ship application materials via Federal Express:
Use Federal Express, Airborne, UPS, or DHL Priority overnight for delivery by 10:30 AM to ship all your passport application materials to (Please include label for UPS/DHL account a $5.00 label fee if not included with request.) :
