UPDATES: Effective November 1, 2005 all passport
applications submitted via JIDS must be completed
on-line www.travel.state.gov
, so they will contain the required 2D bar-code.
Once completed and the “submit” button
is clicked, and application is created and printed,
applicant must sign and submit with the required
documents to Just-In Document Service, Inc.
Please follow the following instruction
in order to expedite your application for a US passport.
Please
note that as of February 1st, 2004 ALL
customers using DS-11
must appear in person at an acceptance
facility to expedite passport request,
INCLUDING CHILDREN OF ALL AGES.
Previously children under 13 years of
age did not have to appear, this has
changed as an additional step in the
prevention of parental child abduction
and substitution.
As
of November 1, 2004 the Statement
of Consent from non-applying parents
must be notarized. This new requirement
will apply when either form DS-3053,
Statement of Consent, or a complete
written consent letter, is submitted
with an application for a child under
the age of 14.
ANY
SEALED ENVELOPES:Client
must tell clerk to make copy of application
that enclosed in sealed envelope COPY
MUST BE ON OUTSIDE OF ENVELOPE effective
immediately.
Proof of travel within
14 days.
Proof of US citizenship.
Passport application form DS-11
completed online but NOT signed
as the it should be witnessed by
the Acceptance Agent.
Two passport photographs.
One copy of authorization letter.
Actual plus clear photocopy of
driver's license with photo on it.
Government fees: $142.00 ($30.00
cash to the Acceptance Agent + $112.00
check or money order to US DEPARTMENT
OF STATE). (A $ 12.00 Security fee
is being added to the original fees
$ 100.00)
The Acceptance Agent endorses
the Passport Form (DS-11)
and place it in a Government envelope
which should NOT be opened after
it is sealed. (Envelope will not
be sealed without a 14 days proof of
travel). Sealed envelopes from
the US Dept of State must be presented
within 5 business days of being sealed.
JIDS fees will be based upon available
appointments and the fees will range
from 100.00, 130.00 or 200.00

Step 2: Collecting US passport application materials:
1. Proof of US citizenship: (one of
the following)
Old passport of which the
applicant was the bearer.
Original Naturalization
Certificate (US citizens through naturalization).
Certificate of Citizenship
(if born abroad to US parent/s).
2. Parents Proof of Identity:
Valid driver's license with
photo issued more than 6 months ago.
A clear photocopy of driver's
license with photo on it.

a valid photo driver's license or if license was issued LESS than 6 months

Child's Relationship to parent(s)/Guardian: (one of the following)
Foreign Birth Certificate
(with parents names & official translation).
Adoption Decree (with adoption
g parents names).
Court order establishing
Custody OR Guardianship.
4. Parental Application Permission
Documentation: (one of the following)
Please
note the child should appear in person under all cases.
Both parents must appear
together.
One parent appear and submits
second parent's written statement of
consent authorizing passport issuance for the child.
One parent appear and submits
primary evidence sole authority to apply:
Original or Certified copy of Birth Certificate listing only applying
parent.
Foreign Birth Certificate (official translation) listing
only applying parent.
Court order granting sole custody to applying
parent.
Adoption decree, if applying parent is sole adopting parent.
Death
certificate of non-applying parent.
If none of the above documentation
is available, the applying parent/guardian should submit a signed statement explaining
why the non-appyling parent/guardian consent cannot be obtained.
Step
3: Ship application materials via Federal Express:
