Example: If your passport was lost overseas or issued in an emergency situation and you were not able to supply a birth certificate with a raised seal at the time of application then you must include the birth certificate with the raised seal at time of extension. Call our office with the number codes in the amendment page of the passport and we will verify which documents are needed.
