UPDATES: AS OF NOVEMBER 4, 2009 ALL applications
presented to acceptance facilities or JIDS MUST
be done through the www.travel.state.gov
website utilizing the bar-code, then printed, signed
and given to Just-In Document Svc.or to acceptance
facility to seal, then JIDS to process. ANY application
HANDWRITTEN, received by JIDS will be redone with
bar-code by JIDS and charged a service fee of $25.00
in addition to processing fees. Please call us

Also please note:
Name Change-As of today form # 5504 will be used
instead of DS-19 if passport name is changed within
1 year of issue date. If name change is more than
1 year form issue date of passport than form DS-82
must be used.
2 passport size 2x2 passport quality photos with
a white background must accompany the request and
a new booklet will be issued, and the old one will
be returned. This is in addition to the requirements
listed.

Step 1: Collecting US passport application

computer. Double sided is only accepted
on ORIGINAL applications which can be
obtained from an acceptance facility,
such as post office.
Name change can use either US passport
application form DS5504
or DS-82
depending upon when name change occurred.
