NEW UPDATES: Effective November 1, 2005 all passport applications submitted via JIDS must be completed on-line www.travel.state.gov , so they will contain the required 2D bar-code. Once completed and the “submit” button is clicked, and application is created and printed, applicant must sign and submit with the required documents to Just-In Document Service, Inc.

Also please note: Name Change-As of today form # 5504 will be used instead of DS-19 if passport name is changed within 1 year of issue date. If name change is more than 1 year form issue date of passport than form DS-82 must be used.
2 passport size 2x2 passport quality photos with a white background must accompany the request and a new booklet will be issued, and the old one will be returned. This is in addition to the requirements listed.

Step 1: Collecting US passport application materials:
 
  1. Your valid US passport:
 
  2. Name Change Official Document: (one of the following)
     
    Certified Court order.
    Certified Marriage certificate.
    Certified Divorce Decree.
      Please note the following:
  • You have to submit original copy of the above document(s).
  • Document must have raised seal.
  • All official documents will be returned with your passport.
  •      
      3. Letter of Authorizations:
         
        Click for Letter of Authorization (this letter allows Just-In Document Service, Inc. to expedite your passport application.)
     
      4. US Passport Application Form:
         
        US passport application form DS5504 can be completed online or picked up at a Passport Acceptance Facility.
       

    Name change can use either US passport application form DS5504 or DS-82 depending upon when name change occurred.

     
      5. Proof of Travel within 14 days (one of the following):
         
        Copy of flight itinerary from the travel agent or airline.
        Copy of plane ticket.

    Step 2: Authorization Process:
     
    A $ 60.00 company check or money order with the name and date of birth written on it issued to US DEPARTMENT OF STATE, or Just-In Document Service, Inc can issue a check for you and charge your credit card $ 3.00 check writing fee.     Step 3: Ship application materials via Federal Express:
     
    Use Federal Express, Airborne, UPS, or DHL Priority overnight for delivery by 10:30 AM to ship all your passport application materials to (Please include label for UPS/DHL account a $5.00 label fee if not included with request.) :
