Instructions to Expedite a Name Change on Your Current Passport.

Please note: Name Change-As of today form # 5504 will be used instead of DS-19 if passport name is changed within 1 year of issue date. If name change is more than 1 year form issue date of passport than form DS-82 must be used. 2 passport size 2x2 passport quality photos with a white background must accompany the request and a new booklet will be issued, and the old one will be returned. This is in addition to the requirements listed.

Express Service - Instructions Passports processed in 4-5 business days. If you are leaving in less than 14 business days you may use this service. Regular Service - Instructions Passports processed in 6 business days or more. If you are leaving in less than 10-30 business days you may use this service. *If you are leaving in more than 5 weeks contact your local post office for service.
