Proof of Identity Instructions

  • If the applicant does not have a valid driver’s license with photo or if the driver’s license was issued LESS than 6 months ago. Additional evidence is required to establish identity.
  • Submit at least 1 primary and 6 secondary items from the list below:
  • Make sure photocopies of the items are very clear.

Primary ID:

  1. Driver’s license (valid, expired or suspended)
2. Federal, State or Municipal identification card with photo.
3. State ID card.
4. Military ID with photo.
5. Pilot’s license with photo.
6. Employment identification card with photo.
7. Current student identification card with photo.

Secondary ID:

  1. Copy of school; yearbook page with photograph with photo.
2. School transcripts.
3. High School Diploma
4. Collage Diploma
5. Military record.
6. Medicare or health insurance card.
7. Medical record.
8. Marriage certificate and license.
9. Divorce Decrees.
10. Baptismal certificate.
11. Voter’s registration card.
12. Income tax record.
13. Pay stubs.
14. Bank statement
15. Social Security card.
16. Utility bills.
17. Leasing /mortgage documents
18. Vehicle registration or title.
