1: Authorization Process: |

Please note if your passport
is lost, mutilated, damaged or stolen a brief
letter from applicant explaining what happened
to the passport in their own words is required. |

Present the following IN PERSON
to your local Passport Acceptance Agent (in
local Post Office or Court Houses): |

|
|
ANY
SEALED ENVELOPES:Client must
tell clerk to make copy of application that enclosed
in sealed envelope COPY MUST BE ON OUTSIDE OF
ENVELOPE effective Immediately. |

|
- Proof of travel within
14 days.
- Proof of US citizenship.
- Passport application form DS-11
completed online but NOT signed as the it
should be witnessed by the Acceptance Agent.
- Two passport photographs.
- One copy of authorization letter.
- Actual plus clear photocopy of driver's
license with photo on it.
- Government fees: $157.00 ($30.00 cash to
the Acceptance Agent + $127.00 check or money
order to US DEPARTMENT OF STATE).
(A $ 12.00 Security fee is being added to
the original fees $ 115.00)

The Acceptance Agent must be notified
that a passport service will deliver the passport
application material to a Passport Agency. |

The Acceptance Agent endorses the
Passport Form (DS-11)
and place it in a Government envelope which should
NOT be opened after it is sealed. (Envelope
will not be sealed without a 14 days proof of
travel). Sealed envelopes from the US
Dept of State must be presented within 5 business
days of being sealed. JIDS fees will be based
upon available appointments and the fees will
range from 100.00, 130.00 or 200.00 |
